Human Rights Watch says asylum seekers on Nauru and Manus Island should immediately be evacuated.
The organisation is marking five years this week since Australia relaunched its policy to detain boat people offshore.
The group's Australia director Elaine Pearson says it's been five years of too much suffering and the policy has had a devastating human cost.
